The Board of Education has contracted with the National Center for Safety Initiatives (NCIS) to provide this service. The short process is completed online at no cost to you.

Tax Exempt Certificate

The Tisdale PTO is a tax exempt organization. This means that we do not pay NJ sales tax. When you shop for supplies for PTO events, be sure to use this certificate. Sales tax expenses incurred are not reimbursable.

Cash Reimbursement/Check Request 

Use this form to request reimbursement for authorized PTO expenses or to request a check to pay a Vendor. Please fill in all details and attach original itemized receipts/invoices. 

Income Report

Please use this form report cash and checks received at an event.  You can submit this request and then drop off the cash in the front office.  

Start-up Cash Request Form

Please use this form to request any petty cash needed for an event.  Please submit this form at least one week prior to your event.
